Sonoma Valley Mentoring Alliance

The Sonoma Valley Mentoring Alliance (SVMA) was established in 1999 as a public benefit, non-profit corporation to assist in the funding and recruiting of mentor candidates. Although we operate the program within the Sonoma Valley Unified School District, the source of all of our funding comes from grants and donations. In an effort to strengthen bonds between mentors and mentees, we try to offer opportunities for field trips and social events that take the pairs off-campus, where some of the 'best mentoring' happens. Field trips are designed to challenge the individual participants through a supportive environment that fosters trust, teamwork, and self-confidence. Social events involve opportunities to bring mentors and parents together in a safe setting where food or entertainment and food is provided. The Sonoma community is in the forefront of the Mentor Movement. In October, 2000 and October 2001, the Stand By Me Mentoring Program was awarded the distinguished Spirit of Mentoring Award by the California State Mentor Initiative, in recognition of the program's outstanding service and leadership in building a child's link to a better tomorrow. This award was presented annually (from 1999 , 2001) to only four school-based programs and four community based programs in the state of California. Our hope is to be able to expand the program to serve students on the waiting list at every campus in the Sonoma Valley Unified School District. St. Pete Pride

St. Pete Pride, Inc. is a non-profit corporation organized exclusively for charitable, scientific, literary and educational purposes, as a tax-exempt entity under Statute 501C3 of the Internal Revenue Service Code, as amended and in accordance with Florida Statutes. St. Pete Pride was established in 2003 as a grassroots Pride Celebration. In just 5 short years, St. Pete Pride has grown in attendance by 500% and is now recognized as "The Largest Pride Celebration in Florida." The 2007 St. Pete Pride Promenade & Street Festival was estimated at 70,000. The projected attendance for the June 28, 2008 Celebration is 80,000-100,000. St. Pete Pride set a standard for financial responsibility from the very beginning. Our commitment to giving back to the community began the inaugural year in 2003 with an annual commitment to help fund a new home for AIDS Service Association of Pinellas and Hospice of The Florida Suncoast. Pegasus Farm

Pegasus was founded in July 1985 to maximize the potential of persons with disabilities. Today 36 horses and ponies carry 190 riders aged from 3 to 73 to new levels of mobility and self-esteem. We also serve at-risk youth and provide learning opportunities for school and 4H groups on our 120-acre farm. Pegasus Farm is a non-profit 501(c)(3) organization and is fully accredited by the North American Riding for the Handicapped Association (NARHA) that sets safety and quality standards for therapeutic riding nationwide and the Certified Horsemanship Association (CHA). All Pegasus instructors are both NARHA and CHA certified.

Committed Partners for Youth (CPY) is a non-profit mentoring agency servicing youth-in-need in the Eugene/Springfield community. We serve youth (ages 7 to 17) through our partnerships with Eugene/Springfield elementary schools, middle schools, social services agencies and the juvenile court system. CPY was founded in 1991 by a local mother and psychologist, Llew Wells, after struggling for many years with the challenges facing her own son and daughter. She knew in her heart that a mentor could have made a huge difference in their lives. With the help of others, she made it her mission to create this unique local program in order to help other children in need.

BIKUR CHOLIM is one of the leading Jewish health care and social support organizations in Greater Los Angeles. Children, adults, and families who come to Bikur Cholim will find over 40 services and programs to help them lead healthier lives. We represent a heritage of compassion, care, and trust, and an unwavering dedication to those who come to us for help. The Bikur Cholim Holocaust Survivors Assistance Program (HSAP) addresses the health and welfare needs of the most vulnerable surviving victims of Nazi persecution who live in poverty. It is the highest priority of the HSAP that the medical needs of these Survivors be addressed with the utmost dignity and care, and with a realization that time is of the essence for a diminishing population. Bikur Cholim is uniquely suited to provide trusted services to Holocaust Survivors and follows in the time-honored footsteps of community assistance organizations of pre-War Jewish life in Europe. When a Jewish Holocaust Survivor hears the words "Bikur Cholim", they inherently trust us, as we represent a continuation of the life and traditions of their lost home. Each Survivor has a case manager who understands and is sensitive to their culture, past, experiences, and special needs. Holocaust Survivors have a variety of problems specific to their Holocaust experience. Many Survivors who were in concentration camps or forced labor have medical problems directly related to experimentation, harsh living conditions, starvation, disease, and injury. Often, those who were in hiding or able to escape suffered illness or injury that has had lasting effects until today. This is all exacerbated by the debilitating state of poverty and medical issues specific to the elderly as well as by the loss of a spouse, family member, and/or never having children. The escalating costs of health care, home care, custodial care, housing, food, and transportation has an overwhelming impact on those Survivors of limited financial means. The lives of many Survivors have been jeopardized or completely deteriorated due to their impoverished state. Tragically, many Survivors do not receive the life-saving care and attention they need due to lack of funds. The United Jewish Communities, Jewish Federation, Beth Tzedek, and Jewish Family Service estimate that there are thousands of Holocaust Survivors living in poverty throughout Greater Los Angeles. The Bikur Cholim HSAP responds to this crisis by helping pay for home care, health care, medication, food, and transportation. A Survivor's ability to convalesce in a familiar and healthy environment can literally mean the difference between recovery or relapse, life or death. Many medical insurance plans, as well as Government programs, do not provide access to specialists or fully cover medical care, in-home care, medication, and medical equipment. For the neediest of Survivors, this often results in going without surgery or the medications and supplies they need, or even suffering from malnutrition. For those Survivors who have little or no access to transportation, HSAP enables them to maintain a continuity of care with their physicians and health care providers. In 2007 the Bikur Cholim HSAP was awarded an allocation in the amount of $50,000 by the Claims Conference on Jewish Claims Against Germany to be used for the neediest victims of Nazi persecution. As many organizations continue commemorating those who perished in the Holocaust through ceremony and memorials, Bikur Cholim hears the voices of those who have survived and answers their calls for help.

MST Services offers comprehensive assistance with the full development of MST programs by providing program start-up assistance, initial and ongoing clinical training and program quality assurance support services. MST Services grants license agreements to MST programs and provides program development and training services in MST worldwide. MST Services' program development and support consists of a comprehensive package of services designed to do "what it takes" to ensure that your MST program will be successful and sustainable. These services fall into four areas: program start-up including initial staff training, ongoing clinical support activities, ongoing organization support activities, and quality assurance support. The program start-up services include technical assistance and materials designed to produce a program description, projected budget, and implementation timeline. Key critical elements include clear articulation of the target population definition and prioritization process, referral and discharge criteria and processes, recommendations regarding clinical record keeping practices and initial program evaluation planning. The MST Program Developer will visit your community to provide an overview presentation and meet with your community stakeholders to assure the buy-in needed for program success after start-up. Next, staff recruitment assistance includes sample job descriptions, help with advertising, interviewing and selecting staff most qualified to implement MST successfully. Finally, all selected initial staff will complete the 5-day MST Orientation Training. The ongoing MST clinical support we provide has been developed to replicate the characteristics of training, clinical supervision, consultation, and monitoring provided in the successful clinical trials of MST. This program implementation protocol has been refined through extensive experience with communities and providers in numerous sites in the United States and internationally. After start-up, training continues through weekly telephone MST consultation, for each team of MST clinicians, aimed at monitoring treatment fidelity and adherence to the MST treatment model, and through quarterly on-site booster trainings (1 1/2 days each). Our trained MST experts will teach your MST supervisor to implement a manualized MST supervisory protocol and collaborate with the supervisor to promote the ongoing clinical development of all team members. The MST expert will also assist at the organizational level as well. Ongoing organizational assistance aims to overcome barriers to achieving successful clinical outcomes through services that may include a comprehensive business planning process, promotion of the MST program within the broader service community, and developing program-level interventions designed to increase referrals, reduce staff attrition, or restructure program funding mechanisms to increase sustainability. Quality assurance support activities focus on monitoring and enhancing program outcomes through increasing therapist adherence to the MST treatment model. The research on MST has consistently indicated that adherence to the model is critical to achieving reduced rates of recidivism and incarceration. The MST Therapist Adherence Measure and the MST Supervisor Adherence Measure have been validated in the research on MST with antisocial and delinquent youth and are now being implemented by all licensed MST programs. Additionally, new measures of supervisor practices, organizational, and broader systems-level influences on client outcomes are under development and are available to interested MST sites. Assistance to funding organizations: MST Services offers assistance to funding organizations to assure that funding structures are sufficient and the funder's program requirements are compatible with MST program standards. Examples of this type of assistance include providing materials and technical assistance to help with developing practice standards, writing a Request for Proposals (RFP), and reviewing provider responses, if requested. At the funding organization's discretion, MST Services will provide technical assistance to organizations responding to funding RFPs to assure that selected proposals contain the necessary elements and address or remove barriers to implementation. Since 1996, MST Services has learned that the ongoing program support, including orientation training, booster training, program consultation and adherence monitoring, is necessary to prevent program drift and maintain high client outcomes. MST Services grants licenses for the use of the MST treatment technology by client organizations and also contracts for program support services on an annual basis.

It became her mission to bring together the various agencies and stake holders in a safe, healing and collaborative community space. Along with fellow disaster volunteers and a dedicated board, September Space is realizing that dream through the same mutual support and comfort that were as crucial at Ground Zero as they are now. Every day September Space embodies the giving volunteer spirit that reaches out beyond oneself, beyond the normal everyday boundaries to help others in need. The sacrifices these giving people made are seen day in and day out at September Space. Expanding beyond the volunteer and rescue worker, September Space has grown to be known as a trusted, safe place where anyone affected by the attacks of 9/11 can come and seek respite. Committed to long-term support September Space provides a warm, casual gathering place where comfort, support and a diverse menu of programs are easily accessible and free. September Space serves a large and diverse constituency, addressing their needs for alternative approaches to healing, rather than how they are affected by the attacks of 9/11. American Indian Community House

AICH was founded in 1969, by Native American volunteers as a community-based organization, mandated to improve the status of Native Americans, and to foster inter-cultural understanding. Since its inception, AICH has grown into a multi-faceted social support agency and cultural center with a staff of 35 that serves the estimated 27,000 Native Americans in New York City. AICH membership is currently composed of Native Americans from 72 different tribes. Native American migration between urban centers and reservations demonstrates the inter-relatedness of all Native Americans, and from this reality emerges the recognition that our issues and concerns are truly shared. The AICH philosophy is that solutions can be shared as well, AICH provides programs in job training and placement; health services referral; HIV referral and case management services; and counseling programs for alcoholism and substance use. AICH also sponsors programs in cultural enrichment through a performing arts program and the first permanent Native American gallery in New York City.

African Services Committee Inc

African Services has provided HIV counseling and testing to over 50,000 people since 2003. Our clinics are located in large, populous market areas visited by 10,000 people each day. We provide free and confidential rapid testing that yields results in about 20 minutes. Over twice as many people have used ASC's testing services than the original estimate over a period of three years.
